Warning! Major spoilers ahead for Chainsaw Man!

Those who only know Chainsaw Man from the anime trailer and haven't read the manga might just think that the protagonist, Denji, is a mindless slasher. In some sense he is, especially when he transforms. But he's actually just a kid who has a simple dream. When Denji realizes he may never achieve it though, he falls into a delusional state and creates a coping mechanism that plays out in the most disturbing way.

At the very beginning of Chainsaw Man, Denji is forced to live in destitution and severe poverty. His father died in debt, and the amount of money he owed is so extreme that Denji's entire existence revolves around securing the funds to repay them. As such, he has no time to experience what normal teenagers take for granted, nor does he have enough money to live comfortably. Because of this, Denji's dreams are simple. All he wants is a girlfriend and to eat well. The likelihood of this coming true seems improbable until that fateful day when he meets Public Safety Devil Hunter Makima. Because Denji has Chainsaw Man inside of him, Denji wields unimaginable power, and Makima believes that he would be an incredible asset.

As a Devil Hunter, Denji eats food that isn't rotten and sleeps in an actual bed. But he still needs a girlfriend. Ever since laying eyes on Makima, he becomes so enraptured by her that he promises that she will be his first no matter what. Everything he accomplishes as a Devil Hunter from that moment on is therefore in service of that. It seems for a while that Makima might actually reciprocate these feelings... That is until it happens. Makima reveals in chapter 82 that she only cared about the Chainsaw Man inside of him and that when she realized the two couldn't be separated, she immediately resented Denji and became the source behind every bad thing that happened to him in the manga, for she didn't believe Denji deserved to live a normal, happy life.

Although as Chainsaw Man he stands up for himself by slashing Makima to bits, Denji has a disturbing mental breakdown where he performs normal tasks in his monstrous Chainsaw Man form with some entrails wrapped around his neck for good measure. He first goes to a fast-food restaurant in chapter 85 and sits down at a table without saying a word except for some incoherent sounds (and killing employees), implying that he wants to be served. Anyone who doesn't act normally or serve him are immediately slaughtered. Then in chapter 86, he kidnaps a girl who falls into a fit of hysterics due to her unfortunate predicament and forces her to go on dates. First, he makes her get some ice cream with him and then play a dancing game at an arcade.

It's never revealed outright why this is happening, but it's clear that Denji is desperately trying to perform all of the tasks that he wished to experience when he was destitute: Eating good food at restaurants and going out on dates. Exacerbating this need is the belief that he just killed the one connection to that life, so he's trying to force his way in at any cost. But the hideousness of his monstrous appearance as Chainsaw Man and unhuman mannerisms clash in such an alarming way with the environments he's trying to fit in with that it just shows how far he is from actually experiencing these things normally, regardless of how fervently he wants them.

No other manga has come close to what Chainsaw Man achieves in these mere two chapters. Many other characters have mental breakdowns that either lead to them falling into catatonic states fits of insane laughter or cries of terror. Very few involve someone trying to cope with their suffering by creating a delusion of such magnitude that involves them pretending as though everything is okay. And, unfortunately, Denji's suffering is only beginning.
